Support Coordination & Recovery Coaching

Bright Holistic has a team of highly skilled Social Workers, experienced Support Coordinators, and Recovery Coaches. We provide a range of services:

Coordination of Supports:
  • Support Coordination (Level 2)
  • Specialist Support Coordination (Level 3)
  • Psychosocial Recovery Coaching

Support Coordination:
A support coordinator is responsible for coordinating and linking you to a range of services and resources in the community. This may include healthcare providers, social services, employment services, housing assistance, and other resources. The support coordinator works closely with you to identify your needs, develop a plan of care, and advocate for your rights and interests.
  • Supporting you to understand and navigate your NDIS plan
  • Supporting to connect you with supports and services you need to achieve your goals
  • Addressing any barriers you come across in regards to your plan or your supports
  • Ensuring your supports are appropriate and holding them accountable to NDIS standards
  • Supporting you to know your rights and advocating where needed

Specialist Support Coordination:
Specialist support coordination is a higher level of support. The focus is on reducing complexity in the participant’s support environment and helping the participant overcome immediate and/or significant barriers in plan implementation. Specialist Support Coordinators are qualified and experienced to deliver the expert approach which is needed to address a persons complex support needs and/or risks in their environment. All of our SSC are experienced Social Workers.

Recovery Coaching:
Similar to Support Coordination, a psychosocial Recovery Coach supports you to manage your NDIS plan but is also someone who helps you with mental and emotional distress to achieve your unique needs and goals on your healing journey. They focus on the psychosocial aspects of your life, such as relationships, housing, employment, education, and community involvement.
This may involve working with you to identify your strengths, challenges, and resources, and creating a plan that includes strategies for improving self-care, building healthy relationships, and pursuing meaningful activities.
Many recovery coaches have their own lived experience and draw from their own life experiences and recovery to help you achieve your goals and improve your quality of life, while also promoting a sense of empowerment and self-belief.
